All associates require the following documents to be considered for employment with Total Healthcare Staffing:

  • Resume
  • License (signed)
  • Malpractice Insurance
  • Any other professional credentials or professional certificates
  • Education verification
  • High School Diploma/GED
  • Driver’s license
  • Social Security card
  • Proof of citizenship/Passport/Permanent Resident card
  • Infection control education – (Registered Nurses)
  • Two or more professional references
  • Physical exam (signed and stamped by a physician from within 1 year, some within 30 days. Must be on form from hospital where associate is working)
  • 10-panel drug screen (within 30 days)
  • PPD (from within the past year)
  • 2nd-step PPD (from within the past year)
  • Quantiferon (recommended)
  • Chest x-ray with TB form and back-up lab work
  • T-D Tap
  • Titres: Rubella, Rubeola, Varicella, Mumps, T-DAP
  • Backup lab work for titres
  • Flu Shot
  • Vaccinations: Hepatitis B
  • Proof of LiveScan fingerprinting (HHA’s/CNA’s)
